Third-Party Information Request

UQ operates an administrative access scheme to facilitate access to personal information by authorised third-party representatives of UQ students, staff and clinic patients.

Law firms and insurers

Law firms and insurers may apply for access to the personal information of their client (or claimant, where applicable), subject to appropriate authorisation from the individual whose records are requested.

UQ charges an application fee for such requests, along with a per-page photocopying / scanning / collation fee for requests involving 150+ pages. Please refer to section 3.1.3 of the Access to and Amendment of UQ Documents Procedures and the Schedule of Fees and Charges for further information.

All information requests under this administrative access scheme are to be made as per below.

Requirements

1. Complete and submit the request form below (credit card required);

2. Email the following to UQ’s RTI & Privacy Office (rtip@uq.edu.au):

(a) Your receipt number (eg. WP11111111) and/or a copy of your receipt;

(b) Signed authorisation from the individual whose records are requested (eg. General Authority, Notice of Claim, etc);
(c) Covering letter (where applicable).
