Summary of Changes from Previous Version

Updated references to DVC(A) and Fitness to Study Review Committee to add that the DVC(A) or nominee will chair, and the DVC(A) will recuse themselves from the SAAAC meeting if they chaired the Fitness to Study Review Committee which considered the matter referred to SAAAC.
